Some of the specific services provided by MCE’s case managers include plan development, linking you with service providers, monitoring services to assure they meet your needs, assisting you in an emergency, assisting you to obtain and keep benefits, visiting with you to develop your individualized training goals.

Your case manager works with you and service providers to plan and secure services for you each year. If you need additional services, case managers must modify your plan with the State and Service Providers.

One of the many advantages of receiving Case Management services through MCE is the increased opportunities for coordination and communication between the Case Manager and the Program with which you are involved. Several in house meetings are held each week. These meetings are used to discuss your specific needs in order to address them as quickly as they arise.
The close communications between the case manager and the program staff assures that any concerns or wishes you express will be shared with the manager of the program and a solution found as quickly as possible.
Everyone who applies to MCE services will receive a case manager of their choice. If you are already funded by the Medicaid Home and Community-based Waiver, you will receive “waiver” case management which includes a home visit each month. If you are applying for services or are privately funded, a case manager will also be provided to you to assist you with your programming or applying to the State of Wyoming for service funding.
Your MCE case manager is your advocate and will represent and assist you with your desires, dreams and planning your meetings and programming. It is important that you chose a case manager that you are comfortable with. If you ever want to change case management you may contact the Case Management Supervisor, Program Manager or staff to assist you in getting a transition either within MCE or outside MCE started.
